Privacy Policy
Effective date: July 2026 · Last updated: August 2026
VitaleHEALTH Family Center (“VitaleHEALTH,” “we,” “us,” or “our”) respects your privacy. This Privacy Policy explains how we collect, use, and protect information when you visit our website (the “Site”), request an appointment, or otherwise interact with us online. By using our Site, you agree to the practices described in this policy.
Please note: Our website is not intended for sharing detailed medical or health information. Please do not include sensitive health details in website forms. The way we handle your protected health information (PHI) as a patient is governed separately by HIPAA and by our Notice of Privacy Practices, which is provided to you as a patient and available upon request.
1. Information We Collect
Information you provide
When you submit an appointment request, callback request, or contact form, we may collect information such as your name, phone number, email address, preferred location, the service you're interested in, and any notes you choose to share.
Information collected automatically
Like most websites, we may automatically collect limited technical information such as your IP address, browser type, device information, pages viewed, and referring pages. This information is collected through cookies and similar technologies and is used to operate and improve the Site.
2. How We Use Your Information
- To respond to your appointment requests, callback requests, and inquiries;
- To contact you about scheduling, your care, or services you've asked about;
- To operate, maintain, and improve our Site and services;
- To comply with legal obligations and protect the rights and safety of our patients, staff, and practice.
We do not sell your personal information.
3. Your Health Information & HIPAA
As a healthcare provider, VitaleHEALTH is committed to protecting the privacy and security of your protected health information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state law.
How we use and disclose your PHI as a patient — including for treatment, payment, and healthcare operations, and your rights regarding that information — is described in detail in our separate Notice of Privacy Practices. That Notice, not this website policy, governs your protected health information. It is provided to you at your first visit and is available upon request by contacting our office.
4. How We Share Information
We may share the information collected through our Site with:
- Service providers who help us operate our Site and communicate with you (for example, hosting, form and scheduling tools, and email providers), who are permitted to use the information only to provide services to us;
- Legal and safety purposes, when required by law, subpoena, or to protect the rights, property, or safety of our patients, staff, or practice.
We do not sell or rent your personal information to third parties for their own marketing.
5. Cookies & Analytics
We may use cookies and similar technologies to remember your preferences, understand how the Site is used, and improve your experience. You can control cookies through your browser settings; disabling some cookies may affect how the Site functions.
6. Data Security
We use reasonable administrative, technical, and physical safeguards designed to protect the information we collect. However, no method of transmission over the internet or electronic storage is completely secure, and we cannot guarantee absolute security. Please avoid sending sensitive health information through unsecured channels such as website forms or standard email.
7. Third-Party Links
Our Site may link to third-party websites and services, such as our patient portal or online forms. We are not responsible for the privacy practices or content of those third parties, and we encourage you to review their privacy policies.
8. Children's Privacy
Our Site is intended for a general audience and is not directed to children under 13. We do not knowingly collect personal information from children under 13 through the Site. If you believe a child has provided us information through the Site, please contact us so we can remove it.
9. Your Choices
You may contact us at any time to update your contact information or to ask us to stop contacting you for non-medical communications. To exercise your rights regarding your protected health information as a patient, please refer to our Notice of Privacy Practices.
10. Shop & Orders
When you purchase a product through our online shop, we collect the information you enter at checkout — your name, email address, phone number, and shipping address — together with the item(s) you order. We use this information only to process, fulfill, and ship your order, to send order confirmations and receipts, and to answer questions about your purchase.
Payment is processed by Clover, our third-party payment provider. Card details are entered directly on Clover's secure hosted payment page and are never stored on or transmitted through this website. Clover's handling of your payment information is governed by Clover's own privacy policy and industry (PCI DSS) security standards.
Purchases made through the shop are retail transactions kept separate from your medical or patient records. Buying a product does not create a provider–patient relationship, and shop order information is not added to your clinical record or used for treatment.
11. Changes to This Policy
We may update this Privacy Policy from time to time. When we do, we will revise the “Last updated” date above. Your continued use of the Site after changes are posted means you accept the updated policy.
